For example, if you were known then by your maiden name, enter that in the last name field. Also, use your formal name in the first name field, not a shortened version or a nick name (e.g., use Michael instead of Mike). Initial Password For your initial login, please enter your date of birth using a DDMMYY format. Again, use whatever date ZS had while you were with the company.
